It seems like you have several symptoms of autoimmune arthritis and I think the rheumatology consult is absolutely necessary. Xrays are often sufficient to diagnose RA if erosions show up. Always includes the shoulder, arm and hand....most over 50 have it...just some are more severe than others...one of those electrode type test...like they do for carpal tunnel...will tell.
