To answer the previous 2 questions. My pain was pretty severe around the back belt line. I was just starting to have sciatic pain down the right leg - but the most severe pain was in the back local to the L5-S1 region. This was my 1st surgery, but discectomy and fusion had been previously reccomended. I also did yoga, pilates, VAXD, pain killers, epidural injections, massage, chiropractic and other physical therapy over the course of the previous 10 years - with varying levels of relief. My disc degeneration was finally just too much and they were starting to shift begiing to stress the facet joints.

Prior surgeries may make you inelgible for ADR. I'm not sure which ones, ask your doctor.

The only thing my doctor told me was permanantly off my list was bungee jumping. I like to golf and ride my Motorcycle and these activities are off limits for 3 months after surgery. Also I need to avoid anything with twisting for three months.

He did not specifically comment on heavy lifting, but I work out with weights and he did say I could resume this after 2-3 months with light weight. Hope this helps.
