... Hi jeujeu, the plugs have a little flat circular part that rests outside (at the top of) the tear duct; I believe this design is for easy removal (by the doctor, of course!). There are plugs now that can be injected as a liquid/gel into the duct, which then harden inside the duct, so there is nothing sticking out. ... I have severe dry eye and tried the permanent silicon plugs. I had my eye dr take them out after 5 days because I could feel them touching my eyeballs when I moved my eyes to the left or right.
