What has helped is to put a slight lift under my heel and arch, and to cut away the insole of my shoe right under the big toe, to allow the point of my big toe to drop down a bit as I walk. ... Every case and every surgeon are different. You need to talk with him about how he does it and what he's post op instructions are. The surgeon I plan to use IF I do the fusion has said I'd only be off it for two weeks and then I could drive (right foot big toe) and go back to work. He'll use a plate. Some don't. Be sure to query your surgeon with all your questions.

I don't think the plate vs screw is a concern. The plate can be bent at any angle and therefore accomdate various heel heights. I've discussed this with my surgeon as I want to be able to wear a higher heel. As to your 4 questions:
