The people who receive you claim and imput it into the computer don't review the claim, just plug in the numbers and the computer is programed to automatically ajudicate/process it with no decision by... Keep working within the appeals process -- I believe you are right that since they wouldn't disclose the name of an approved provider in a timely manner you had to make the best decision at the time. They might ask you, though, if you asked the provider if they were an approved provider (that part's your responsibility).
