... there are conversion tables for this kind of dosing and your doctor will probably start one dose lower than recommended because your pain receptors will not be saturated like the fentany ones, so you will be more sensitive to the oxycodone in oxycontin. ... Hey Scrapper, I have a copy of the Kadian conversion handbook made by the manufacturer, According to it, the 50ugh would be replaced by 150-200 mgs of Kadian. However, equinalgesic tables are just guides to prevent docs from ODing a patient and to get you in the ballpark. The rule is to always side on caution and patient safety. ... According to my equianalgesic chart, 50 mcg of fentanyl is the appropriate dose for a 100 mg Morphine BID. You have to remember that everyone responds differently to opiates. This may work for you or you may need to increase. Talk with your Dr. As far as the BT meds, it does take a little while for the patch to kick in so most Dr's advise of taking them to keep your pain... (2 replies)
