Yes, your TSH is very, very high, but the number that's much more important than that is the free T4 at .4. That's no doubt below the lab range; most people begin to have symptoms just when the free T4 drops below the center of the range. Yours has fallen all the way off the bottom.

You don't necessarily need to be "alarmed", but you will do best with a competent thyroid doctor who will dose you according to your free T4 and free T3 levels.

Your weight will probably not show much change just by taking thyroid medicine. You'll still have to work at it, but improved thyroid levels will help you get rewarded from the effort.
