I'm starting to become frustrated with how little good advice exists on Silent Reflux/GERD. Can anyone offer advice? I'm 35 years old and have been on Protonix for about 2 years. I started with 20 mg for one year and then amped up to 40 mg in year 2. I originally started taking it because I would experience choking sensations while eating 3 times per week and severe mucus, but I never had any of the classic symptoms of acidic taste, trouble sleeping, etc. An endoscopy showed redness and inflammation of the esophagus and stomach but nothing severe. Fast forward 2 years and I feel like I now do have trouble sleeping, sore throat, and choking episodes are up to about 6 times per week.

I'm starting to dispute the theories that my stomach has too much acid but nobody really gives a detailed plan how to get off ppi's and I don't trust physicians any more. I have had apple cider vinegar make heartburn improve. I also just drank it mixed with water with dinner and my dinner went down smoothly for the first time all week, albeit I only ate one meal all day because I was dreading the mucus feeling. Would the following plan of attack work?

1. Start taking digestive enzyme DGL, probiotics and yogurt daily, and drinking apple cider vinegar daily.
2. Start introducing HCl pills into my diet to recreate an acidic stomach
3. Reach the 30 day mark on the anti-anxiety med Lexapro
4. Gradually take less of the PPIs as I investigate bacteria's ability to stop the reflux events and see if the anxiety, stomach and diet issues were to blame not the acid.

I'm really confused how to switch my plan of attack in steps safely. Lots of advice is on the web but nobody outlines a plan. Thanks for any ideas.

You are wise to be thinking of a plan to get yourself healthy. You should be aware that anti anxiety meds can cause acid reflux, and likewise acid reflux meds can cause anxiety. So you don't have a clear picture of your health whilst taking both these drug groups.

Taking Apple cidar vinegar has helped your digestion because you have insufficient acid in your stomach and adding an acid can help digestion. However, do not take ACV or HCI supplements if you are planning to reduce the PPI because when you reduce PPIs you can get RAHS - rebound acid hypersecretion which is excessive acid and you don't want to be taking anything that will be adding even more. Probiotics and DGL are ok. Perhaps consider getting off one drug first and before stopping the other as it can be hard on the body.
