roxychick- I rinsed only with water because the glycolic acid makes your skin sensitive and then using the washcloth, however gently, irritates the skin even more. I think you're right that your skin gets a little worse before better, mine did. Obviously, if it gets lots worse or stays that way, you can always call your dermatologist's nurse to double check!

Amaru- DON'T skip immediately to MD Forte lines II or III without dermatologist or licensed esthetician approval. The differences in glycolic acid strength between the three lines may not seem like much, but it can really make you skin go crazy! Follow the directions. If it says to try the low dosage (line I) for a month before moving up to line II, then do that. It takes a while for your skin to get used to any glycolic acid, and moving too quickly will not let your skin adjust. If you skip lines (without being told to by someone who knows), your skin could react badly, and then you'll never know if the MD Forte could've really worked for you (had you used it like the directions state). I totally understand your temptation to slather on stronger stuff in order to reduce your acne, but it's not the way to go in this instance.

Also, I used it twice a day, but if your skin is super-sensitive, you may want to use it once a day and a super-mild cleanser the rest of the time. I'm not sure about exfoliating. It probably depends on your skin. I wouldn't exfoliate the first couple of days you use it because your skin will be adjusting. Then, I'd exfoliate very gently, maybe with a washcloth rather than a scrub. I wouldn't exfoliate at the same time that you use the MD Forte, though. Try doing one in the morning and the other in the evening.
