I have a question along the same lines. Had the ACTH stim test last week, and initial draw for cortisol was 21 (range 8-23). The second one was 25. My binding globulins are messed up, so most of my "total" hormones are high or in the upper part of the range...while the "frees" are low or low normal.

My cortisol should have risen more than that, right?

And I know it's not diagnostic, but I also had the 24 hr urine done (mostly to RULE OUT Cushing's), and my cortisol levels were 2.5, 3.0, and 2.2 (range 8-70).

Would someone please comment and point me in some direction? Thanks!

Thanks for your response.

I have had saliva tests, which I assume measures the free cortisol. It was average-ish in the morning (in range but on the lower end) but dropped off to 1/2 of the lowest value of normal at noon and in the evening. By midnight, it was still lower than the noon and evening "spits," but it was (barely) in range b/c the range is so low then.

Does that make sense? :confused:

Also, what did you mean when you said that I'm the only exception that you've seen? Are you talking about my "normal" serum cortisols with the low UFCs? Yeah, I'm pretty much a mess... :)

Thanks for your help! I think I'm going to have the growth hormone stim test before I take anything. Hopefully, it won't be long...
Oh, my primary doc acknowledged adrenal problems after seeing that test but tried to treat it with nutritional changes/supplements. Didn't work. He says 1.5 in the morning and less than 1 the rest of the day is what he considers "failure." Things never improved, so he sent me to an endo who's doing MORE tests...
Do you have any comments on an ACTH level of 18 (range 9-52)? Normal or no? Done at the same time as the first serum cortisol (21; range 8-23)...but 1/2 hour before the stim result of 25...
