Yeah, lyme's disease is a possibility. Also Chiari can cause problems like those you specify depending on what the malformation is compressing.

Over what period of time did the symptoms you mentioned develop? ALS would not develop all over the place in any short time frame. Also widespread twitching and twitching in muscles that are not clinically weak, points away from ALS. Loss of overall muscle tone does eventually occur in ALS but generally not all at once. Typically function loss starts in a single muscle usually in a distal muscle such as a hand or foot or in the tongue in bulbar onset cases.

While I don't hear ALS in your symptoms there definitely appears to be something going on. It may or may not be neurological but starting with your neurologist seems to be a good plan. Good luck and let us know how things go.
