I am sorry your Mom needed the surgery for the bones. Unfortunately for any dementia elders, the surgery and hospitalization definitely make the elder decline further due to confusion and/or by anesthesia for surgeries. My father-in-law who has late stage of Alzheimer's got worse after surgeries and hospitalization.
I feel for you.
Your Dad may not be able to accept the reality and I understand that. She will get worse and I hope hospice makes her comfortable. You may want to get a personal caregiver who can watch her all the time so she would not keep falling in the wrong way. Sometimes the nursing home lets you hire a caregiver for 2 or 3 hours per day so someone can watch her closely.
