My hematologist says that she sees patients with B12 levels in the 400-600 with major symptoms including fatigue, muscle aches, etc. She wants my levels to stay above 800 or so. I get monthly injections and supplement daily with the other vitamins.

So don't count out the B12 thing yet. If I were you, I'd add sublingual B tablets to your regime - they work very well for many people. You can get them online or at any specialty vitamin stores.
