RESULT: MRI CERVICAL SPINE WITH CONTRAST

INDICATION: Post laminectomy syndrome. Lumbar spondylosis. Thoracic
spine and low back pain.

COMPARISON: None

TECHNIQUE: Multiplanar multisequence MR images of the cervical spine
were performed on a 1.5-T magnet. 5 cc IV gadolinium was administered.

FINDINGS:
Status post anterior cervical discectomy and fusion C4-C6.

Neural elements: Normal.

Pre-vertebral soft tissues: Normal.

Lung apices: Normal.

Head and neck structures: Normal.

Alignment: 2 mm grade 1 C3-C4 retrolisthesis. Straightening of the
cervicothoracic spine.

Bones: Bone marrow edema inferior endplate C3 and C6.

Paraspinal muscles/tendons and ligaments: Normal.

Craniocervical junction: Normal.

Occipitoatlantal (O-C1) articulations: Normal.

Atlantoaxial (C1-2) articulations: Normal.

Subaxial c-spine:
C2-3: Uncovertebral hypertrophy. Posterior disc osteophyte. No central or
neural foraminal stenosis.
C3-4: Retrolisthesis. Disc bulge. Central canal 5 mm, marked stenosis.
Moderate bilateral neural foraminal stenosis.
C4-5: No central or neuroforaminal stenosis.
C5-6: No central or neuroforaminal stenosis.
C6-7: Tiny disc bulge. No central or neuroforaminal stenosis.
C7-T1: Normal.



MRI OF THE THORACIC SPINE WITH CONTRAST.

TECHNIQUE: Sagittal T1, T2 and STIR, axial T1 and T2-weighted images of
the thoracic spine were obtained on a 1.5 Tesla magnet. 5 cc IV
gadolinium was administered.

FINDINGS:

Alignment: Straightening of the thoracic kyphosis.
Spinal Canal: Developmental stenosis absent.

Bone: Vertebral body heights are preserved.

Sacroiliac joints:Incompletely visualized.

Muscles: Normal.

Soft Tissues: Normal.

Visualized lungs: No visualized abnormality

Abdomen/Retroperitoneum: Normal.

Spine:
Perineural cysts, left T6 (8 mm CC) and right T7 (13 mm CC).
Tiny disc bulge T10-T11, T12-L1. No central or neural foraminal stenosis.


MRI OF THE LUMBAR SPINE WITH CONTRAST.

TECHNIQUE: Sagittal T1, T2 and STIR, axial T1 and T2-weighted images of
the lumbar spine were obtained on a 1.5 Tesla magnet. 5 cc IV gadolinium
was administered.

FINDINGS:

This report assumes 5 non-rib bearing lumbar type vertebral bodies. The
lowest of these is labeled as L5 in this report, and is identified by the
iliolumbar ligament and lumbosacral angle.

Alignment: 2 mm L2-L3 retrolisthesis, 4 mm L3-L4 retrolisthesis, 5 mm
L4-L5 anterolisthesis; grade 1.
Spinal Canal: Developmental stenosis absent.

Bone: Chronic degenerative endplate wedging of T12 vertebral body. Modic
type II degenerative changes of the lumbar spine.

Sacroiliac joints:Incompletely visualized.

Nerves: Conus medullaris is normal in location terminating at L1-2.


Muscles: Normal.

Soft Tissues: Normal.

Abdomen/Retroperitoneum: Subcentimeter nonenhancing cyst right kidney (
series 11 image 24)

T12-L1:
- Intervertebral disc: Disc desiccation. Tiny disc bulge.
- Posterior Elements: Mild degenerative facet hypertrophy.
- Central canal: Patent.
- Neural foramina: Right normal. Left normal.

L1-2:
- Intervertebral disc: Disc desiccation. tiny disc bulge.
- Posterior Elements: Mild degenerative facet hypertrophy.
- Central canal: Patent.
- Neural foramina: Right normal. Left normal.

L2-3:
- Intervertebral disc: Disc uncovering. Disc bulge.
- Posterior Elements: Mild degenerative facet hypertrophy.
- Central canal: Mild central stenosis.
- Neural foramina: Right mild stenosis. Left normal.

L3-4:
- Intervertebral disc: Disc uncovering. Disc bulge.
- Posterior Elements: Mild degenerative facet hypertrophy.
- Central canal: Mild central stenosis.
- Neural foramina: Right moderate stenosis. Left moderate stenosis.

L4-5:
- Intervertebral disc: Disc uncovering. Disc bulge.
- Posterior Elements: Mild degenerative facet hypertrophy.
- Central canal: Mild central stenosis.
- Neural foramina: Right moderate stenosis. Left moderate stenosis.

L5-S1:
- Intervertebral disc: Right paracentral disc protrusion.
- Posterior Elements: Mild degenerative facet hypertrophy.
- Central canal: Patent.
- Neural foramina: Right mild stenosis. Left normal.

Impression
IMPRESSION:
C-spine:
1. Status post anterior cervical discectomy and instrumentation C4-C6
with junctional degenerative disc disease at the C3-C4 level above.
Minimal C3-C4 retrolisthesis with reactive bone marrow edema inferior
endplate of C3.
2. Marked central spinal and moderate bilateral neural femoral stenosis
at C3-C4.

T-spine:
1. Perineural cysts, left T6 (8 mm CC) and right T7 (13 mm CC).
2. No central spinal or neural foraminal stenosis.

L-spine:
1. Minimal L2-L3 and L3-L4 retrolisthesis and grade 1 L4-L5
anterolisthesis.
2. No central spinal or neural foraminal stenosis
