MRI Spine lumbar & sacral:
Posterior surgical change with focal defect in the lumina bilaterally at L4/L5.
The tip of the shunt lies within the thecal sac at L4/S1 level & runs obliquely across the right L4/5 neural foramen & impresses on the right L4 nerve root.
There is apparent clumping of the nerve roots with empty sac sign.
L5/S1 annular tear & central dorsal disc protrusion indents the ventral aspect of thecal sac.
The conus medullaris is unremarkable.
L4-5 Modic type ll endplate degenerative changes.
No significant spinal canal stenosis.

[B]CONCLUSION[/B]
The LP shunt impresses on the right L4 nerve root at the exit foramen.
Arachnoiditis.
L5-S1 annular tear & central posterior disc potrusion
