... I have tried everthing you state above and then some. I even had a colonoscopy. Any new drug that comes out for IBS, I'm there. I hardly ever eat white bread and my diet consists of mostly chicken, fruit, whole grains, veggies and plenty of liquids. Once a week I may eat a hamburger or a steak but thats it. Mega colon is a major thing, however (Google it) - and if you had it - I think you would be seriously sick right now (like fever, etc), so don't start freaking out, thinking it's end of the world - the human body is pretty resourceful, etc. But you should be seeing a doc on a regular...
