In terms of the optometrist and/or opthalmologist...yes, they can tell if certain things are wrong by looking inside your eyes. Much of what they can tell has to do with high cranial pressure, high blood pressure throughout the body and also small vessel disease.

As for what you're complaining about, it sounds like you might be having some trouble either with something inside your ear whose pain is being referred to the back of your head (which happens) or you are having trouble with your mastoid process (which is a bone behind the ear) OR, there is some other trouble which is hard for me to figure out because I haven't been able to examine you.

I think it might be a good idea to have the doctor check it out because what you describe kinda sounds like mastoiditis--i.e. infection and/or inflammation of the mastoid bone behind the ear which can spread in the head area. I'm not saying that mastoiditis is what it is, but since that's a possibility, it's probably a good idea to get it checked out.
