If I had as much trouble with a statin as your husband did, I doubt that I'd be willing to try a diffferent one. But his numbers aren't good, so I expect he'll want to do something. How much niacin is he taking? Be sure that he has liver enzymes tests at regular intervals, even if it is an OTC niacin he is on. He may be able to keep his cholesterol under control with it if he has no problems with liver enzymes. Niacin has been used for cholesterol control for about 50 years. Unfortunately, it does raise homocysteine, but if he can't take statins, it may help somewhat.

My total cholesterol was 275 at its highest I believe. I got it down to around 247 with diet only, then began taking Enduracin, a sustained-release niacin, starting with just 250 mg daily and building up to 750 mg along with additional dietary changes. I did get my TC down to as low as 190 on the 750 mg at one point, though with slacking off on the diet, it was up to 239 again last time. My HDL was up to 85, though.

If your husband is able to tolerate a lot of legumes in the diet, that helps, too. And the closer to a vegetarian he can become, the better as far as cholesterol.

He is scheduled for another test in six months so we will keep an eye on the OTC time-released niacin and his liver enzymes. Now that he has started limited exercising that may help as well. We are considering a trip to England in the spring, but some place warm with a beach in January is sounding better.

One more quick question. I don't remember seeing this mentioned. What is phenotype? It says Phenotype Type IIA Borderline right under the lipid profile.

[/QUOTE]If he has started taking OTC niacin tablets and the doctor isn't aware of that, I'd suggest you call the doctor and see if he wants to schedule the next liver enzyme test sooner. I've read that the usual recommendation (which is the same one my doctor followed) was to schedule a liver enzyme test three months after initiating treatment or after any increase in dosage and six months thereafter if staying on same dosage. Your husband's doctor may have a different schedule, depending upon the dosage your husband is taking, any other health issues, etc., but I just think it might be wise to let the doctor know that niacin could be a factor in that decision.

Jane: I don't know what the Type IIA is for certain. However a phenotype is the apparance of an individual which results form the interactions of the person's genetic makeup and his/her environment. An example, if a childs genotype (genitic makeup) includes the gene for brittle bones, the brittle bones are the phenotype. Other words the phenotype is the obvious symptoms (appearance). I would suspect the phenotype IIa means that your husbands genetic makeup and the actual appearance of increased cholesterol puts him in the IIa category. Kind of if a person has a big rear end , it is (most likely) due to genetic and environment (food intake); therefore, their phenotype is TYPE big rear end... Have a good time somewhere warm. The stress reduction will do you more good than any medication could possibly do.
