Thanks for your replies, ladies! Here's a little more about the situation. After IUI #3 was unsuccessful, we had a consult with the RE. He suggested dh go see the urologist, as his counts and numbers have been all over the place. Sometimes count is fine, sometimes it's very low. Sometimes motility is ok, sometimes it's really low. You get the picture. So we did IUI #4, and his numbers were actually pretty good...to the point that we considered cancelling with the urologist. At my urging, we went to the appointment. DH was NOT prepared for what the physical exam involved (poor guy!). DH has not had any pain, so he was surprised when we learned he does have a varicocele. Urologist, who does specialize in fertility, tentatively recommended the surgery. He just want to do a second SA and an ultrasound first. Andvari, I will make sure that the hormone levels have been checked. Thanks for that recommendation. We should have IUI #5 within the next few days and actually go back to the urologist Monday to review the 2nd SA and have a testicular u/s. I think the urologist expects that we'll make a decision about the surgery at that time. I want to get all the info and bring it to our RE before making any decisions. I really want to understand how much this could improve our chances of success with IUI or IVF. I'm 36 1/2, so taking 3-6 months off from trying while dh recovers from surgery and gets the numbers up seems like a long time. I do think dh is a relatively good candidate, as I think his lowest numbers were 10-12 million (although MUCH lower after the washing for IUI).

SlyFoxy- where have you been and what is going on with you?
festivus - in reading your post I can't help but wonder why your doctor hasnt gone done the route of IVF. Typically that is a good way to go if the guy has issues. It takes a lot of the guessing out. Or is this something you guys have already discussed? If not I would suggest maybe seeing about doing IVF if insurance covers it once before taking the 6 months off for a surgery. Hopefully though you get extra lucky and this final IUI attempt works :)
