The sporadic nature makes me think this could be associated with an autoimmune disease, which often involves peripheral neuropathy. Tanning can exacerbate it, especially if lupus, because UV light exposure, sunlight or tanning beds, triggers DNA release from sun damaged skin cells. The causes of PN in general are: vitamin B12 and folate deficiency, autoimmune diseases like lupus, diabetes, alcohol excess, and hypothryodiism. So a thorough blood test investigation for all those should be ruled out, with history being most important on the alcohol link. I would get any and all blood tests done to rule these out that have not been done within last year. A previous negative ANA, for example, could be positive now. TSH over 3.0 indicates hypothyroidism. B12 orally isn't always absorbed and may need to be by injection. There are so many nuances in PN, and a treatable cause must be sought! Not enough to just treat symptoms initially without a full work-up to determine cause.
