... Hey Scrapper, I have a copy of the Kadian conversion handbook made by the manufacturer, According to it, the 50ugh would be replaced by 150-200 mgs of Kadian. However, equinalgesic tables are just guides to prevent docs from ODing a patient and to get you in the ballpark. The rule is to always side on caution and patient safety. I know Fentanyl will stay in our syatem about a day after you take off the patch, since you have experienced WD from switching from the patch before that's easy to explain. ... rGazer, I wouldnt give up on MSC just yet, there really arent enough options not to give it a fair chance and their is definitely room to adjust your dose.. Most conversion guidelines suggest the doc calculates the conversion and than starts at half of the estimate. You really aren't near the ball park as far as a comparable dose. ... (3 replies)
