That PSA of 7.15 would be in the normal range for PSA produced by healthy cells if your prostate is about 91 cc. Actually, a PSA of 3.3 would be abnormally low for a prostate that big. One rule of thumb is that each 10 cc of healthy prostate tissue (including BPH) accounts for about 1 unit (ng/ml) of PSA.

What's next? Sometimes a multiparametric MRI is done; one of its benefits is that you get a very accurate report on the size of the prostate, and that is important for understanding prostate cancer risk. I suspect the risk is low in your case, because of the apparent size of your prostate, but it might be worth getting more and more certain evidence.
