While your PSA is certainly elevated, and apparently not enlarged, there could be other causes for elevation. One common cause, in addition to benign enlargement, is infection/inflammation; calcifications are another, less common cause, and there are others. Unlike the US, multiparametric MRIs are readily available and commonly used for staging prostate cancer in London, so you are quite fortunate. The mpMRI will give you a definitive measure of the size of the prostate; sometimes the internal (finger) exam is misleading.

You could get a biopsy under a general, though, with numbing agents that should be used ahead of time, the procedure should not be that bothersome unless many cores are taken. After a mpMRI, there is much better targeting of the biopsy cores, so not so many are needed.

The fact that your PSA measurements over two years are the same is encouraging because prostate cancer doubles at a rate that is individual for each of us, and your readings do not reflect this. Cancer is still possible, as other causes, such as infection, could be a major influence on the movement of PSA levels, masking a PSA increase. I very much like use of an mpMRI at this point!
