I think your instincts are correct, in at least scheduling a repeat PSA/free PSA test about 3 months from the recent one. While the increase from 3 to 5 could be significant, getting another one would be good. (When mine increased from 3 to 4 last year, I got a second one 3 months later- it went up a little more- and I followed that by a biopsy.)

It's possible that an infection is part of the increase in PSA results. Also, the DRE should not be done prior to the blood taking for the test (as this stimulation of the prostate can cause the levels to be higher than otherwise). Also, sexual activity within 2 or 3 days prior to a PSA test can cause the results to be higher.
