Galebear, after reading your post I found myself more then alittle angry.
I went through pretty much the same thing, although my doctors at the time I believe were playing guessing games, no tests at all but sent me to pt, I was in that office at times once to twice a week. That was over 2yrs ago, now I have chronic pain & just went through shoulder surgery, I can't help but get angry at the fact that if they would have taken me seriously & did a full exam & diagnostic testing I could have possably avoided it getting to this point.
So take my advice see another doc. for another opinion, took me three to get someone to get the ball rolling. I had xrays that did not show anything at all. Even an MRI did not show the full extent of the damage, when the surgery on the shoulder was done more was revealed. Xrays do not show everything.
Now as far as pt goes I was sent twice before surgery, the first for they did not even know the shoulder had problems, so it made everything worse, so did the second. I do believe that pretty much all doctors try this measure. My surgeon told me that surgery is only done when all other methods are exhausted, pt, injections, meds, etc.. Of course I would assume that if they found something & knew it had to be surgically corrected that would be the exception.
I feel for you because I went through that, very little pain control, getting no where with the docs, & my life falling a part. If I had to do it all over again I would have not allowed myself to be treated that way, I even had one doc tell me it was stress & depression. Yea, ok, did not know it could cause me to loose use of my arm & vomit every morning in pain. Don't except this garbage, you deserve to have good treatment & to not have to suffer. There are alot more testing measures out there & definantly alot more pain control methods. Good luck & let us know what happens, Sammy
