[B]TL;DR: Since this past November...painful urination (dysuria) made worse by stronger stream, constant dull ache in urethra (sometimes concentrated in certain area), random sharp pains in urethra, urethra painful to touch (not just at tip), painful erections/masturbation/ejaculation, involuntary "clenching" just before urine exits, urinalysis/urine culture showed nothing off, gonorrhea/chlamydia test negative, CT scan showed no stones, a (rushed, doc was on tight schedule) cystoscopy revealed nothing amiss, 6-week course of Cipro brought no relief. Issues with intentionally slowing urine stream (for months) and the tip of my urethra "sealing" shut from substance - painfully blocking urine flow - may be of relevance to symptomology. Reckless masturbation may also be of import.

What can be done to help me with my symptoms? What caused/is causing my symptoms? Please help!! I'm desperate for answers and effective treatment. The pain is really bad :'(

[B]MY SYMPTOMS (they started this past November; they have progressively worsened since):[/B]
-A constant dull ache felt all along my urethra that often seems concentrated in certain parts.
-Sharp pain while voiding (it's a feeling of painful constriction/pressure) that increases the stronger my stream is.
-An involuntary "clenching" right as the urine is about to exit (stopping the flow) that is very hard to resist and leads to a feeling of pain.
-Random sharp stabs of pain in my urethra, and sometimes elsewhere in the shaft of my penis.
-Unbearably painful tightness when experiencing an erection, as though something is being stretched too tight or compressed/expanded to the point of pain. Ejaculation causes a "shooting" sort of pain that starts somewhere at the base of my penis and travels up toward the tip. Needless to say, masturbation is painful and I no longer do it.
-My urethra, when touched (by manipulating the shaft of my penis), is painful and tender. The very tip of my urethra is generally not painful to touch but hurts during and after voiding.

[B]NOTE:[/B] I'm transgender and have been pursuing hormone therapy (blocking testosterone and introducing estrogen). Although I will pursue bottom surgery in the future, I still have functioning male genitalia at present. Also, I still have a prostate (and have no ovaries, etc.).

[B]POSSIBLY RELEVANT BACKGROUND INFORMATION:[/B]
-Prior to the aforementioned symptoms, I started to experience random blockages where the tip of my urethra was "sealed" shut by some sort of substance. This led to a feeling of extremely painful pressure as the urine sought to exit, but could not. Sufficient pressure eventually built (over a few seconds) for the blockage to be dispelled. This happened quite a few times.
-In response to the blockages, I started to consciously slow down the flow of my urine so as to save myself from the pain. I slowed my stream like this for a few months before abruptly stopping this. When I tried to relax and urinate normally again, it hurt and I started to experience everything listed in the "Symptoms" section.
-I had sexual contact with someone who I later learned was diagnosed with a yeast infection.
-I had a long period of time where I did not masturbate (a year) and then abruptly resumed masturbating regularly, in what may be described as a vigorous fashion. I remember one time in which I experienced, during ejaculation/directly after ejaculation, shooting pain that traveled from somewhere below the base of my penis to the tip.
-Around the time I started experiencing my symptoms I was hospitalized for a brief period of time (for unrelated reasons). While hospitalized, my entire body felt painfully rigid for the duration of my stay. This feeling never quite left me.
-I developed balinitis, and also hemorrhoids, for the first time in my life a few months into experiencing my symptoms.

[B]PROCEDURES/ACTIONS TAKEN:[/B]
-Multiple urinalysises that did not show anything was off
-A gonorrhea/chlamydia test which came back negative
-A CT Scan which did not show any indication of stones
-A cystoscopy which did not reveal anything out of the ordinary (however, the procedure was very rushed as the doctor was on a very tight schedule. it did not feel satisfactory.)
-A roughly 6 week course of antibiotics (Ciprofloxacin) which did little if anything to help
-I have seen 2 separate urologists, neither of whom has any clue what is going on with me.

[B]MEDICATIONS/SUPPLEMENTS (current):[/B]
-Estradiol (2 mg tabs three times a day)*
-Spironolactone (100 mg tabs twice a day)*
-Gabapentin (600 mg cap. twice a day)**
-Tamsulosin (0.4 mg cap. once a day)**
-Colace** (1 pill each day)
-Zyrtec** (1 pill each day)
-Vistaril (25 mg tabs PRN)**
-Mirtazapine (two 15 mg tabs each night)
-Hydrocortisone Acetate suppositories (25 mg, one inserted each night)
-Clotrimazole and Betamethasone Dipropionate Cream (as needed for balinitis)
-Vitamin B12 complex, D, E**
-Magnesium and Zinc**
-Fish oil
-Psyllium**

[B]MEDICATIONS/SUPPLEMENTS (prior):[/B]
-Aripiprazole (long-term)
-Bupropion (long-term)
-Duloxetine (a few days)
-Paroxetine (a few days)
-Estradiol Valerate (intramuscular injections in thigh; long-term)
-Sumatriptan (taken once)
-Lorazepram (taken a few times, 15 mg tabs PRN)
-Alprazolam (for a few months, 1 mg tabs PRN)

[B]OTHER MEDICAL CONDITIONS:[/B]
-Psoriasis
-Neck, shoulder, and back pain/tenseness (I see a chiropractor every month)
-Migraines
-Issues with constipation (largely relieved at this point)
-Diagnosed depression
-Tinnitus
